Underlying health conditions for Covid vaccine – what conditions are included for group 6?

COVID vaccines have been handed out to almost 25 million people in the UK since the start of December. The Government has opened up coronavirus vaccine bookings to priority group six, which includes people aged 16 to 64 with underlying medical conditions. What conditions have been added to the list?
